Do you use something like The Babysitter's Notepad when you hire a sitter, or expect your babysitter to come prepared?

I am a babysitter and I have one of those in my babysitter bag that I bring to the house.

 

So I think that both the babysitter and the parents should have something like it!

I don't know if you NEED a babysitters note pad.
tell the first time babysitter:
just make the sitter aware of any health concerns/ allergies.
the routine.  eg.  bed time (what time, a bottle, special blanket etc.)
give key to house if need be.
rules

and write down for every time you go out.  even if it is the same sitter:
parents cell #'s
At least 2 emergency contact #'s [eg.  close neighbors (in proximity and to the family)]
